Stateline Oral & Maxillofacial Surgery



After establishing a plan of treatment, we will provide you with an estimate of costs prior to your surgery. If you have insurance, you will be required to pay the patient's estimated payment at the time of surgery unless prior arrangements have been made. If you do not have active insurance, you will be required to pay the charges in full on the date of service.

For your convenience, we accept cash, check, Visa, MaserCard, Discover and Care Credit

It is the patient's responsibility to provide our office with complete and accurate insurance information. Payment for deductibles, co-pays and non covered benefits are due at the time of service. All patients are fully responsible for all charges regardless of insurance coverage. We will gladly submit an insurance claim for rendered services. You will be required to read and sign our financial policy.
